Organizations are under growing pressure to adopt and deploy artificial intelligence tools. But what happens when those tools fail, cause harm, or operate in unexpected ways? This presentation examines how to identify and respond to AI incidents, including bias, privacy breaches, malicious attacks, and control failures. It will cover the core elements of an effective AI incident response plan, governance considerations, root cause analysis, and practical steps to strengthen preparedness. The program will conclude with a tabletop exercise that guides participants through the response to an evolving AI incident.
